An Rx Symbol and a Stethoscope for a Family Physician
This letterhead says medicine without words. A blue corner holds the Rx symbol that patients recognise from prescriptions, and a stethoscope curves along the footer. It suits a family physician, a primary care doctor or a general practitioner who writes prescriptions, notes and letters and wants a page that looks like a doctor’s stationery.
The Blue Corner and the Rx Symbol
The page is 8.5 x 11 inches, portrait, one page. A blue shape fills the upper corner and carries the Rx symbol, a capital R with a slashed tail. It is a picture that you can replace with a clinic logo using Replace Image. Keep the Rx if you plan to write prescription style notes on the page.
The Stethoscope Footer
A drawing of a stethoscope curves along the foot of the page and holds the contact lines inside its loop. It is a graphic, so it stays the same when you edit text. The footer lists a street address, a phone number with an email and a website. Check that each line fits inside the loop after you edit.
A Mismatch in the Sample Text to Correct
The sample degrees line reads M.S. (Ayurveda), Ph.D. (Ayurveda), Medical Officer, followed by Dept. of Periodontics & Oral Medicine. The department belongs to dentistry and does not match a family physician or the Ayurveda degrees. It is placeholder text left over from another design. Replace the whole line with your own qualifications and department before you use the page.
Name and Date
The header reads Dr. First Last Name. At the top right, a Date line is ready for use. Type your name as it appears on your registration, and keep the date format the same in every letter.
Notes Patients Expect From a Family Physician
- Prescription notes and medication instructions.
- Letters for a hospital admission or discharge.
- Certificates for fitness, illness and travel.
- Referral letters to a specialist.
- Follow up reminders and health advice sheets.
A Reminder About Formal Prescriptions
Prescriptions are regulated documents in most countries and may require specific fields, registration numbers and security features. A decorative page is not a substitute for an approved prescription pad or an electronic system. Use this page for letters and notes, and follow your local rules for prescriptions.
Layout Tips for the Rx Page
You can edit all of the text lines. Adjust the font size if the degrees line is long. Leave a little space between the Rx symbol and your first line of text so the header does not look crowded.
Printing the Corner and the Footer
A colour laser handles the blue corner well. If your printer cannot print to the edge, a small white border will appear around the shapes, and that is normal. A print shop can supply full bleed sheets for daily use.
