Replace the Logo and School Name First
This doctor ID card is filled with sample data that does not quite match. The front and back show a medical shield logo with the name SPRINGFIELD HIGH SCHOOL and the motto Excellence, Integrity, Compassion, while the card is for a physician, and the back says the bearer is a licensed physician employed by St. Mary’s Medical Center. A hospital doctor’s card with a school’s name would be confusing. Replace the organisation’s name, the motto and the address with your own hospital or clinic on both sides before you print.
Doctor Card Front: Navy Curve and Shield
The card is portrait, 2.125 x 3.375 inches. The top is deep navy with a large white curve and a bright blue ring. A medical shield with wings and a cross sits at the top, with the institution name and motto in white. A big white circle holds the photo placeholder, with a navy and blue ring around it. Below, DR. MICHAEL ANDERSON is in bold navy capitals, with a blue pill reading DOCTOR. A table lists Department: Cardiology, ID No: SMH-1028, Contact: +1 (202) 555-4789 and Email: [email protected]. A long barcode picture finishes the front.
Doctor Card Back: Dates and Licence Note
The back is deep blue with a large faint word DOCTOR at the top and the same shield logo. A white arch holds a table with Blood Group: A+, Date of Birth: March 21, 19XX, Issue Date: January 1, 20XX and Expiry Date: December 1, 20XX. A paragraph says: This card certifies that the bearer is a licensed physician employed by St. Mary’s Medical Center. If found, please return to the hospital address and phone number. A QR code and a signature line close the card.
What a Doctor’s Card Should Show
| Item | Note |
|---|---|
| Full name and title | Use Dr. only if the person holds the title |
| Department or specialty | For example Cardiology or Paediatrics |
| ID or licence number | As required by the hospital or medical board |
| Photo | A clear, recent portrait |
| Expiry date | Matching the contract and licence |
Do Not Use This Card to Claim Credentials
A card that says a person is a licensed physician is a serious claim. Only issue it to doctors who are actually licensed and employed by your organisation. Misusing a medical ID is a crime in many places. This template is for hospitals, clinics and medical schools that issue genuine cards to their own staff.
Matching the Back Text to Your Facts
Edit the paragraph on the back to name your own organisation and give its full address and phone number. Make sure the wording is accurate. If the doctor works across several sites, say so, for example “employed by the Metro Health Group.”
Photo and Data Handling
Click the placeholder and choose Replace Image. Keep personal data to a minimum. If your hospital does not need a blood group or a date of birth on the card, replace those rows with Employee ID and Valid From.
Printing Physician Cards
Use PVC cards for daily wear, printed on both sides with a lanyard slot. Replace cards at the end of each contract. Keep a register of cards issued.
For a nursing staff card, see the nurse ID card. For a staff card in a business setting, see the corporate professional ID card. More are in ID cards.
